Qualifications

  • Minimum High School Diploma
  • Valid driverโ€™s license and reliable means of transportation

Duties and Responsibilities

  • Provide support and assistance to clients and home care aides
  • Manage large caseloads efficiently and prioritize duties
  • Maintain and update clientele files and required documentation
  • Build professional relationships with case managers and case management units
  • Help expand business through word-of-mouth and networking
  • Conduct home visits in Skokie and surrounding suburbs

Skills and Abilities

  • Ability to work cooperatively with co-workers and external organizations
  • Strong case management and task prioritization skills
  • Ability to travel within Cook County regularly and outside as required
  • Excellent verbal and written communication skills
  • Strong documentation and reporting skills
  • Understanding of caregiver and participant needs
  • Proficiency in Microsoft Office, especially Excel
