Our Community Care Program (CCP) provides essential in-home care services to eligible seniors and individuals with disabilities, helping them maintain independence and continue living safely in their own homes.

We work in partnership with the Illinois Department on Aging, Illinois Department of Human Services, and leading Managed Care Organizations to ensure our clients receive the support they need. Qualified individuals may be eligible to receive services at no cost.

Who Qualifies?

To be eligible for CCP services, individuals must generally:

  • Be 60 years of age or older
  • Be a resident of Illinois
  • Be a U.S. citizen or legal resident
  • Meet asset and care requirements
  • Have applied for Medicaid

Eligible individuals are assessed by their local Care Coordination Unit (CCU), which determines care needs and assigns service hours.

Flexible, Personalized Care

Once approved, clients receive a customized care plan designed to support their daily needs. Services may include assistance with personal care, household tasks, and overall well-being.

Clients also have the option to choose a trusted family member to serve as their caregiver, allowing for a more comfortable and personalized care experience.

Support Beyond Seniors

Individuals under 60 may also qualify if they have a disability, are at risk of nursing home placement, and meet Medicaid and care requirements.
